This ecological study examined how produce prescription (PRx) program design characteristics influence participant outcomes — including fruit and vegetable intake, food security, and perceived health — across 39 GusNIP-funded programs in the United States. Results showed that programs providing higher incentive amounts to fewer participants, offering transportation assistance, and investing in staff training were most strongly associated with improved participant outcomes. These findings offer practical guidance for designing more effective produce prescription programs and highlight the importance of program structure — not just incentive availability — in driving meaningful health improvements.
